During puberty, insulin doses (basal and bolus) need to be ratcheted up on a regular basis.  Occasional "super highs" are going to happen, but ketones should not.  The presence of ketones indicates a severe lack of insulin.  Forgetting (or neglecting) to take insulin is not something to take lightly.
